If you need to make any changes or additions to your form, please email us with the information you would like updated. All duplicate forms will be deleted.
If you are selected for camp registration, you will be notified via email and given 1 week after notification to make your payment before your spot is given to the next person on the list. Registration is not complete until both this form and payment are received.
