All children enrolled at Dodge Nature Preschool must have up-to-date paperwork on file. Returning families must renew paperwork once each calendar year. All paperwork is due by noon Friday, August 4 .
Please fill out each of the following FOUR forms:
Form #1- Emergency Contact Form (link takes you to a new page)
The Emergency Contact Form provides us with contact and emergency information to keep your child safe at preschool. Please check carefully to ensure phone numbers and email addresses are entered correctly. ALL information is REQUIRED by state licensing. Please fill the form out COMPLETELY. (Emergency contacts can not be the child’s parents, and dentist is required.)
Form #2- Permission and Release Form (link takes you to a new page)
The Permission and Release Form allows us to take hikes on the Nature Center grounds and provides information about your child’s and your family’s needs and preferences.
Form #3- Immunization Records (link is a pdf download)
You may either download an immunization form to fill out and return to us. Or you can request a copy to be faxed from your child’s health care provider. (fax: 651-554-8444) Or you can upload a copy from your child’s ‘my chart’ into the form below. (Please be sure the child’s name appears on each screenshot.)
If you have a notarized immunization exemption, we must receive the hard copy. We cannot accept a scanned or uploaded copy.
Form #4- Health Care Summary (link is a pdf download)
This form must be completed by your child’s health care provider. Most often, the health care provider will then fax us the completed form to the number listed (651-554-8444). You may also upload a copy to the form below.

Tuition Express Enrollment Form (link takes you to a new page)
Tuition Express is our autopay system. Forms can be completed online or you can download and print the form if you prefer. Tuition payments will be automatically deducted from a checking account on the fifth of each month. This is the preferred way to pay tuition. Returning Families can complete the Tuition Express Renewal Form (link takes you to a new page).
Lunch Bunch Interest Form (link takes you to a new page)
Use this form to indicate your preference for Lunch Bunch.
Morning Bunch Interest Form (link takes you to a new page)
Use this form to indicate your preference for Morning Bunch.
After Bunch Interest Form (link takes you to a new page)
Use this form to indicate your preference for After Bunch.
Emergency Care Plans (when applicable)
Most health care providers have an emergency plan for severe allergies and asthma. These forms can be sent to your doctor, if needed (severe allergies or asthma – pdf). They must be completed by the health care provider. Please upload a copy of your child’s completed form below.
