Summer Enrollment Paperwork

Paperwork for enrolled summer families

Completing your child’s Summer Enrollment is a three-step process.


  1. Submit your payment at MyProcare.com (link opens a new window). You will use the same email address to log in as you used when you applied for summer. Payment due ASAP.
  2. Submit your child’s Emergency & Permission Form (link opens a new window)
    The Emergency & Permission Form provides us with contact and emergency information to keep your child safe at preschool. Please check carefully to ensure phone numbers and email addresses are entered correctly. Form due ASAP.
    If your child is a current Dodge Nature Preschool student who WILL NOT return to preschool for the 2026-27 school year, you DO NOT NEED to fill out ANY forms.
  3. Submit your child’s Immunization Records (link is a pdf download) & Health Care Summary(link is a pdf download) This form must be completed by your child’s health care provider. The Deadline for these Health Forms is Monday, May 18.
    If you have a notarized immunization exemption, we must receive the hard copy. We cannot accept a scanned or uploaded copy.

Emergency Care Plans 
These forms are for children with severe allergies, asthma or other medical conditions. Most health care providers have  their own version of an emergency plan for severe allergies and asthma. We prefer these forms. If needed, you can request your child’s doctor send these forms: severe allergies or asthma (pdf). They must be completed by the health care provider.
